Organic Program
Organic forms, documents and instructions
The program is providing the forms in multiple formats.
- Microsoft Word and Excel documents can be completed electronically for mailing or emailing, but have larger file sizes and slower download times.
- Most of the Adobe Acrobat (PDF) documents also can be completed electronically for mailing or emailing and have smaller files sizes and faster download times. But some older forms must be printed and filled out by hand or with a typewriter.
Please remember that you should retain a copy of all forms submitted for your records which must be maintained 5 years.
